Photograph
Some scholarship applications may request a photograph to
be used for publicity purposes after winners are chosen. A
recent color or black and white photograph, no larger than
four by six inches, should accompany your application package
(if requested). Do not enclose candid snapshots or baby pictures!
Federal Income Tax Information
Organizations that award scholarships to students based on
financial need may request a copy of you or your parent(s)
federal income tax return form 1040. Your parent(s) federal
income tax form will be used to verify your family’s
income.
